Overview

Beginning with a presentation of the evidence-basis for Fetal Infant Mortality Review (FIMR) as a community-level quality improvement strategy for maternal-infant health and health equity through multi-sector team review of the socio-cultural, economic and health system factors associated with individual cases of fetal and infant death. Reviews produce recommendations for program and policy changes that are subsequently implemented and monitored. The FIMR process incorporates elements necessary for population health-equity improvement: focusing quality improvement efforts on groups having worse outcomes, tailoring strategies to the circumstances specific to marginalized populations, incorporating the voice of group members, and configuring primary care for accessibility by individuals who have been historically mistreated within the healthcare system. Findings at the local level inform state performance objectives related to infant mortality reduction efforts, and are translated into federal action through Title V Maternal-Infant Needs Assessment and related interventions. With technical support from the National Center for Fatality Review and Prevention, 175 FIMR programs are in operation across the country, and a national Disparities Work Group is laboring to adapt fatality review tools for focused action in high-disparity communities.
