Physician and APP Advance Care Planning Training

A course for physician and APPs to gain valuable information and skills development to aid them in advance care planning discussions with patients.


Tuesday, April 15, 2025, 7:00 AM - 8:30 AM, Bronson Gilmore Center, Kalamazoo, MI

1. Describe the differences of Michigan advance care planning forms, advance directive, DNR or MI-POST
2. Initiate and lead an ACP conversation with patients
3. Identify the ACP resources available to patients
4. Transcribe a detailed ACP Note
